COMPASSION WITH "BRIDGE TO CARE”
The Philadelphia Protestant Home’s home and community-based
program, Bridge to Care, provides home services to seniors
in need in the greater Northeast and surrounding communities
of Philadelphia. The program allows underserved elderly
citizens of lower income to remain in their homes while
maintaining quality of life and dignity. This is accomplished
through a broad range of services that address social and
medical issues, safety and security of the home environment
and access to established community services for seniors.
The
major program components include:
A quarterly
home visit by a case manager/social worker to assess the
need of community services, to suggest and/or refer community
services or to advise and consult on issues related to quality
of life.
•
A monthly home visit by a nurse for purposes of health assessment
relative to the home environment and to provide advice and
consultation on health-related issues.
•
A 24-hour monitoring and response service to ensure communication,
accountability and safety – 24 hours a day, 7 days
a week.
•
Pastoral counseling and health education seminars for program
recipients and their caregivers.
In
addition to the basic Bridge to Care program, the following
services are also available for a moderate fee to seniors
who do not participate in the basic program.
•
Home modification and/or repair related to safety, accessibility
and quality of life provided by trusted PPh personnel.
•
Housekeeping services.
