When you walk into Pathways Healthcare Center, PPh's 126-bed health care center, images of warmth and beauty greet you at every turn - the lobby's gentle waterfall, the rich cherry paneling, and the large illuminated aquariums. Outside the scene is just as tranquil - splendid gardens wind through the property, delicate trees rustle in the breeze. A holistic approach to resident care is provided by a dedicated team of professionals who evaluate residents' nursing, psycho-social, emotional, medical, and spiritual needs. The interdisciplinary team collaborates to ensure the resident is receiving the optimal level of care while following the mission and values of PPh. A rehabilitation coordinator assists the resident through the continuum of care and into the community by assessing the resident's level of care, communicating needs to families, and coordinating discharge plans.

The first step in every treatment regime at Pathways is creating a safe and familiar environment for our residents. The unique design of our rooms provides each resident with a private living space. We offer both private and semi-private rooms. Each semi-private room has a window offering personal space and privacy. We encourage residents to bring personal possessions and mementos to their furnished rooms to help make Pathways feel as much like home as possible.


Short Term Rehabilitation

Short-term rehabilitation in our skilled/sub-acute nursing facility helps residents recover from a hospital stay. It provides individualized rehabilitation/nursing therapy for: accidents, strokes, hip fractures, joint replacement, amputations, and other skilled nursing needs. Our goal is to build residents' strength and skill levels and help them adapt to new challenges as they progress through treatment toward the fullest life possible. For your personal comfort, we provide televisions and phones in our short-term rehab rooms.

Long-Term Care

Long-term care in our nursing facility is offered for residents who require the very best services available. At Pathways, we believe that individuals have heathcare needs that change over time. We provide compassionate care to help uplift residents so that they can enjoy a high quality of life. The facility is secure, the staff cheerful, and the environment bright and stimulating.
