At
Pathways, we believe that individuals have heath care needs
that change over time.
Pathways
is our 126 bed, state-of-the-art health care center built
in 1998 on the campus of The Philadelphia Protestant Home
(PPh).
A holistic
approach to resident care is provided by a dedicated team
of professionals who evaluate residents’ nursing,
psycho-social, emotional, medical and spiritual needs. The
interdisciplinary team collaborates to ensure the resident
is receiving the optimal level of care while following the
mission and values of PPh. A rehabilitation coordinator
assists the resident through the continuum of care and into
the community by assessing the resident’s level of
care, communicating needs to families and coordinating discharge
plans.
The
first step in every treatment regime at Pathways is creating
a safe and familiar environment for our residents. The unique
design of our rooms provides each resident with a private
living space. We offer both private and semi-private rooms.
Each semi-private room has a window offer personal space
and privacy. We encourage residents to bring personal possessions
and mementos to their furnished rooms to help make Pathways
feel as much like home as possible.
